CCHP Member Services

CCHP has a team of friendly bilingual Member Services representatives ready to answer questions about your plan benefits, provider network information, and procedure for using the plan. We encourage you to inform us right away if you have questions concerns or problems related to your covered services or the care you receive.

CCHP Member Services is available to take your calls from 8:00 AM to 8:00 PM, 7 days a week. You can reach this office by calling toll-free at 1-888-775-7888. TTY users can call 1-877-681-8898.

CCHP Member Services representatives will answer your questions about:

  • Covered services
  • Making address or phone number changes
  • Primary Care Physician changes
  • Enrollment or Disenrollment procedures
  • Appeal and Grievance rights and procedures
  • Medical care when you are traveling
  • The quality of care you are getting
  • Mental Health Benefits
  • In-Network Provider Access
  • Claims Processing
  • ANY other questions or concerns regarding the CCHP Program

If you are a member of our CCHP Senior Program (HMO) or CCHP Senior Select Program (HMO SNP) with questions about your Medicare Part D prescription drug coverage you may call Member Services from 8:00 AM to 8:00 PM seven (7) days a week.

Nurse Advice Line

While your physician's office is the best resource for your health questions, they may not be available all the time. CCHP's 24/7 Nurse Advice Line is available get advice from a registered health professional, even on evenings, weekends or other times when your physician is not available.
