CCHP Medicare Advantage HMO plans Overview

CCHP Senior Program HMO and CCHP Senior Select Program HMO are Medicare Advantage HMO plans offered by Chinese Community Health Plan (CCHP), a California-licensed health plan with a Medicare contract. CCHP contracts with over 300 doctors, including primary care physicians and specialists from our medical group, Chinese Community Health Care Association (CCHCA). We contract with Chinese Hospital, St. Francis Hospital, California Pacific Medical Center, St. Mary's Medical Center, Seton Medical Center, and other providers and institutions to provide quality care to our members.

Our Medicare Advantage HMO Plans offer all of the benefits covered under Original Medicare, as well as Medicare Part D prescription drug coverage, plus more.

To become a member, you must be entitled to both Medicare Part A and Part B, and live within CCHP's service area.

CCHP Senior Program HMO and CCHP Senior Select Program HMO are offered by Chinese Community Health Plan (CCHP); a Medicare Advantage Organization, an HMO with a Medicare contract. Anyone who has Medicare Parts A and B may apply to CCHP Senior Program HMO; anyone who has Medicare Parts A and B and full MediCal benefits may apply to CCHP Senior Select Program HMO, including the disabled. You must live in the CCHP service area for respective plans in which you enroll. Eligible Medicare beneficiary may enroll in CCHP Senior Program HMO during specific times of the year. You must use CCHP plan providers, except for emergency care, urgent needed care when our network is not available or out-of-area dialysis care. If you obtain any other care from non-plan providers, neither Medicare nor CCHP will be responsible for the costs. For CCHP Senior Program HMO, you must continue to pay your Medicare Part B premium and any other applicable Medicare premium(s). These plans include Medicare Part D prescription drug coverage and are only available to CCHP Senior Program HMO and CCHP Senior Select Program HMO members. Our plans use a formulary, which lists covered drugs. In most cases, you must obtain covered drugs at one of our network pharmacies listed in our pharmacy directory or through our mail-order service.
