CCHPs marketing mix includes a range of initiatives from grassroots community outreach to traditional broad based media. Supervisor of Case Management

Direct a combined Case Management and Disease Management Program to assure that members have access to quality, cost -effective health care and assists in the assessment, coordination, and monitoring activities necessary to accomplish this. The Pharmacy Technician will work closely with the Pharmacy Services Manager, Medical Director, and/or Clinical Services Director in one or more of the following areas: Formulary Management, Disease Management, Pharmacy Audit and Recovery, Medication Safety and Management, Medicare Part D Operations, State Health Programs Pharmacy Operations, and Prior Authorizations. Duties include, but are not limited to, interacting with pharmacy personnel, physicians, and members to resolve medication-related questions or issues as well as participating in clinical programs and projects to ensure the safety and effectiveness of medication utilization.
