Web Based Inquiry
Eligibility inquiry can be obtained by using the CCHP "Web Based Inquiry" portal. Access to the CCHP "Web Based Inquiry" portal is available only for contracted CCHP health care professionals and facilities. Please click on the above "Web Based Inquiry" link to log-in with your UserID and Password.
If you need assistance with web access, please call Provider Services at 415-955-8800, ext. 3215 or 3221, Monday through Friday, 9:00 AM to 5:00 PM.
If you are not a contracted provider or do not have web access and would like to check member eligibility, please call Member Services at 415-834-2118 between 9:00 AM to 5:00 PM Monday through Saturday.
