Claims Status Inquiry

The following information is for use by CCHP health care professionals and facilities only.

Web Based Inquiry

CCHP contracted health care professionals and facilities can check the status of their claims using the CCHP "Web based inquiry" portal. Please click on the above "Web Based Inquiry" link to log-in with your UserID and Password. After checking online, if you have a question about a claim, please call the Claims department at 415-955-8800, ext. 3322.

If you need assistance with web access, please call Provider Services at 415-955-8800, ext. 3215 or 3221, Monday through Friday, 9:00 AM to 5:00 PM.

If you are not a contracted provider with CCHP or do not have web access and would like to check claims status, please call the Claims department at 415-955-8800, ext. 3322.

Submitting Claims Electronically

CCHP prefers claims be submitted electronically. For information, please call our claims Department at 415-955-8800, ext. 3322.

Submitting Paper Claims

All paper claims must be submitted using a CMS 1500 form (for professional providers) and a UB-92 form (for institutional providers).
