About CCHP
Chinese Community Health Plan (CCHP) is a health care plan (or HMO) delivering culturally sensitive, bilingual managed care to the San Francisco community since 1986. In addition to offering plans for employers and individuals, CCHP is contracted with Medicare to offer Medicare Advantage plans, including Part D.
CCHP is committed to serving our community and is devoted to delivering the highest quality health plan to the people and organizations we serve.
CCHP currently contracts with one medical group, Chinese Community Health Care Association (CCHCA). Each CCHP member selects a primary care physician (PCP) from the CCHCA panel. The primary care physician coordinates all care provided to the CCHP member, including referrals to specialists and arrangements for medically necessary hospitalizations.

Winner of the 2007 National Committee for Quality Assurance (NCQA) "Recognizing Innovation in Multicultural Health Care Award" for our diabetes self-management program to improve diabetes care in the Chinese community.
In 2006, CCHP received the National Committee for Quality Assurance Award "Recognizing Innovation in Multicultural Health Care" for our efforts to reach across cultural divides and provide enrollees in San Francisco with culturally and linguistically appropriate health care services.
From 2001 to 2004, Consumer Reports Guide to California Medicare HMOs rated the CCHP Senior Program (HMO) as the "Best Overall Value" Medicare HMO in San Francisco County.
2000 AAHP Ellis J. Bonner Community Leadership Award
In 2000, CCHP received the American Association of Health Plans Ellis J. Bonner Community Leadership Award in recognition of the distinguished contribution that CCHP made to enhance the quality of life to the Chinese community.
